Objectives: To analyze the association between serum levels of estradiol, testosterone, SHBG, and the free androgen index (FAI) with metabolic syndrome (MetS) in perimenopausal and menopausal female healthcare workers.
Subjects and methods: A cross-sectional descriptive study was conducted on 231 perimenopausal-menopausal female healthcare workers aged 45-55, currently working at several health facilities in Hanoi and Thai Nguyen province, from January 2025 to April 2026.
Results: The MetS group had significantly higher testosterone and FAI levels than the non-MetS group with geometric means of 0.27 nmol/L versus 0.19 nmol/L (p = 0.025) and 0.64 versus 0.36 (p < 0.001), respectively. Conversely, SHBG levels were significantly lower in the MetS group compared with the non-MetS group, with geometric means of 41.27 nmol/L and 54.04 nmol/L, respectively (p < 0.001). SHBG levels tended to decrease, while FAI tended to increase, with an increasing number of MetS diagnostic criteria (p=0,001 and p=0,002, respectively).
Conclusion: SHBG and FAI are two endocrine factors significantly and consistently associated with MetS, and may serve as reference markers for assessing the presence and severity of MetS in perimenopausal-menopausal female healthcare workers.
