Objective: To evaluate IgE and IgG4 responses to house dust mite allergens Dermatophagoides pteronyssinus (d1) and Dermatophagoides farinae (d2), analyze their correlations with total IgE, and describe immune response phenotypes based on combined IgE–IgG4 profiles.
Methods: A retrospective cross-sectional study was conducted on 107 patients who underwent allergy testing at Vinmec Times City International Hospital between June 2025 and June 2026. Allergen-specific IgE and IgG4 to d1 and d2 were measured using the ImmunoCAP Phadia 250 system, while total IgE was measured on the Cobas e801 platform. Descriptive statistics and Spearman correlation analysis were performed, with p < 0.05 considered statistically significant.
Results: Positive IgE rates for d1 and d2 were 92.5% and 92.3%, respectively. IgG4 concentrations were generally low for both allergens. A very strong positive correlation was observed between IgE to d1 and d2 (ρ = 0.94, p < 0.001). Moderate positive correlations were found between IgE and IgG4 for the corresponding allergens (ρ = 0.503–0.565, p < 0.001). IgE to d1 and d2 also showed strong correlations with total IgE (ρ = 0.724–0.801, p < 0.001). The predominant immune phenotype was IgE-positive/IgG4-negative, followed by IgE-positive/IgG4-positive.
Conclusions: House dust mite sensitization was highly prevalent in the study population. IgE responses to d1 and d2 were strongly correlated, whereas IgG4 showed moderate correlations with IgE. Simultaneous assessment of IgE and IgG4 provides additional information on immune response patterns to house dust mite allergens and may support future studies investigating their clinical utility in allergic diseases.
