ASSESSMENT OF THE TRIGLYCERIDE–GLUCOSE INDEX AND ITS ASSOCIATION WITH CLINICAL AND LABORATORY CHARACTERISTICS IN PATIENTS WITH GOUT

Abstract

Background: Gout is a common chronic metabolic disorder closely associated with insulin resistance. In Vietnam, the triglyceride–glucose (TyG) index has been increasingly recognized as a simple and low-cost marker for assessing insulin resistance; however, studies evaluating this index in patients with gout remain limited.

Objectives: 1. To determine the TyG index in patients with gout at Hue University Hospital. 2. To investigate the association between the TyG index and selected clinical and laboratory characteristics in patients with gout.

Subjects and Methods: A cross-sectional descriptive study was conducted on 87 patients with gout at Hue University Hospital from January 2026 to July 2026.

Results: The median triglyceride concentration was 1.81 [1.22–2.55] mmol/L and the median fasting plasma glucose level was 5.21 [4.98–5.67] mmol/L. The mean TyG index was 4.62 ± 0.43, and 48.3% of patients had TyG values above the threshold for insulin resistance. The TyG index differed significantly between patients with and without abdominal obesity (p < 0.001) and between those admitted with or without an acute gout attack (p = 0.039). The TyG index showed a positive correlation with serum uric acid (r = 0.221; p = 0.040) and total cholesterol (r = 0.388; p = 0.001), and a negative correlation with HDL-cholesterol (r = –0.215; p = 0.045). No significant association was observed between the TyG index and age, sex, or hypertension status (p > 0.05).

Conclusion: The mean TyG index in this study was 4.62 ± 0.43 and was associated with abdominal obesity, overweight status, acute gout attacks, and levels of cholesterol and uric acid in patients with gout. The TyG index may serve as a useful indirect marker for early identification of insulin resistance in the comprehensive management of patients with gout.
