INITIAL RESULTS OF SELECTIVE RENAL ARTERIAL EMBOLIZATION USING A LIPIODOL–ABSOLUTE ETHANOL MIXTURE FOR THE TREATMENT OF RENAL ANGIOMYOLIPOMA

Abstract

Purpose: To evaluate the efficacy and safety of selective renal arterial embolization using a Lipiodol–absolute ethanol mixture for the treatment of renal angiomyolipoma (AML).

Materials and Methods: This retrospective descriptive study included 30 patients with renal angiomyolipoma who underwent selective transarterial embolization at Bach Mai Hospital between July 2022 and August 2025. Indications for embolization included ruptured AML, tumor diameter ≥4 cm, or symptomatic lesions (flank pain or hematuria). Technical success, clinical success, procedure-related complications, and imaging follow-up were evaluated in all patients.

Results: Thirty patients were included in the study. All patients (30/30) completed the 3-month follow-up, while 18/30 patients completed the 8-month follow-up. The mean tumor diameter decreased from 7.0 ± 1.8 cm before treatment to 5.8 ± 1.5 cm at 3 months (17% reduction) and further to 4.9 ± 1.4 cm at 8 months (30% reduction). Technical success was achieved in 30/30 patients (100%), while clinical success was achieved in 29/30 patients (96.7%). One patient experienced persistent mild flank pain, and one patient developed a renal artery pseudoaneurysm rupture 6 months after embolization.

Conclusion: Selective transarterial embolization using a Lipiodol–absolute ethanol mixture is a safe and effective treatment for renal angiomyolipoma, providing high technical and clinical success rates with significant tumor size reduction during follow-up.
