SKIN BARRIER REPAIR CARE IN THE MANAGEMENT OF CHRONIC SKIN DISEASES: A SYSTEMATIC REVIEWDiseases: A Systematic Review

Abstract

Introduction: Skin barrier dysfunction is considered a core factor in the pathogenesis of many chronic skin diseases. However, interventions to restore the skin barrier have not yet been consistently implemented in clinical practice in Vietnam. Objective: To synthesize existing evidence regarding skin barrier restoration care protocols in the management of chronic skin diseases. Methods: A systematic review was conducted following PRISMA 2020 guidelines and registered on PROSPERO (CRD420251017829). Searches were performed across four databases—PubMed, Ovid, Wiley Online Library, and the Cochrane Library—and data were synthesized using a narrative synthesis approach. Results: A total of 20 studies were included, involving at least 1,343 participants (based on the 17 studies that reported sample sizes). The studies focused primarily on three groups of chronic skin diseases: inflammatory conditions (n ​​= 16), autoimmune diseases (n = 2), and circulatory disorders (n = 2). The findings indicate a shift from the use of simple moisturizers to structured care protocols that incorporate gentle cleansing, bioactive moisturizers, and guidance on treatment adherence. Based on this evidence, the study proposes a three-step care framework comprising: (1) standardized skin cleansing; (2) disease-specific skin barrier restoration; and (3) combined specialized treatment. Conclusion: Existing evidence suggests that skin barrier restoration care is increasingly being implemented through structured protocols, although the specific interventions vary across disease groups. The proposed three-component care framework may serve as a reference for developing and standardizing care protocols for patients with chronic skin diseases in clinical practice.
