CLINICAL, RADIOLOGICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF INTERTROCHANTERIC FEMUR FRACTURES USING PFNA AT HOSPITAL 19-8

Abstract

Objectives: To evaluate the clinical and radiological characteristics, surgical outcomes, and functional hip recovery in the treatment of closed intertrochanteric femur fractures using Proximal Femoral Nail Antirotation (PFNA) at Hospital 19-8

Subjects and Methods: A prospective cross-sectional study was performed on 29 patients with closed intertrochanteric fractures surgically treated with PFNA from January 2024 to December 2025. Clinical features, AO/OTA classification, TAD index, and reduction quality were recorded. Functional outcomes were assessed using the HHS at 1, 3, and 6 months postoperatively.

Results: The mean age was 72.5 ± 8.4 years, with 55.2% being female. High rates of comorbidities were observed (hypertension 65.5%, diabetes 34.5%), with 41.4% having multiple diseases. Unstable fractures (31-A2) predominated at 58.6%. The mean operative time was 54.4 ± 7.8 minutes, with minimal blood loss (85.6 ± 12.5 ml). The mean TAD was 18.2 ± 3.4 mm; 96.5% achieved acceptable reduction. The mean HHS significantly improved from 65.2 ± 5.1 (1 month) to 86.5 ± 7.2 (6 months) (p < 0.05). At the 6-month follow-up, excellent and good results reached 86.2%, fair 10.3%, and poor 3.5%. No infections or mechanical failures were recorded.

Conclusion: PFNA is an effective option for intertrochanteric fractures in elderly patients, especially those with multiple comorbidities. This method provides stable fixation, facilitates early rehabilitation, and achieves high rates of favorable outcomes.
