INITIAL OUTCOMES OF HOLMIUM LASER ENUCLEATION OF THE PROSTATE (HOLEP) VIA TRANSURETHRAL ENDOSCOPIC APPROACH FOR BENIGN PROSTATE HYPERPLASIA AT 19-8 HOSPITAL

Abstract

Objective:
To evaluate patient characteristics, perioperative parameters, changes in voiding function, urodynamic outcomes, and quality of life, as well as perioperative complications in patients undergoing Holmium Laser Enucleation of the Prostate (HoLEP) at 19-8 Hospital.

Materials and Methods:

A total of 25 patients diagnosed with benign prostatic hyperplasia (BPH) with prostate volume >80 gram were included in this descriptive case series. All patients underwent clinical evaluation, International Prostate Symptom Score (IPSS) assessment, informed consent was obtained prior to surgery. The following parameters were recorded: PSA, urinary flow rate, prostate volume, operative time, intraoperative blood loss, need for blood transfusion, catheterization time, length of hospital stay, and perioperative complications.

Results:

From November 2024 to March 2026, 25 HoLEP procedures were performed. The mean age was 70,1 ± 6,3 years. The mean prostate volume was 92,5 ± 7,6 g (80–140). The mean operative time was 70 ± 6,6 minutes (60–120). The mean catheterization time was 1,88 ± 0,42 days. International Prostate Symptom Score (IPSS) significantly decreased from 25 preoperatively to 11 at 1 month and 9,2 at 6 months (p < 0.001). The maximum urinary flow rate (Qmax) improved from 6,8 to 19,8 mL/s at 6 months (p < 0.001). Post-void residual urine (PVR) decreased from 190,5 to 18,2 mL at 6 months (p < 0.001). Quality of Life (QoL) scores improved from 4,0 to 1,5 at 6 months (p < 0.001).

Conclusions:

The initial outcomes of HoLEP at 19-8 Hospital, Ministry of Public Security, demonstrate favorable results. HoLEP is a safe and effective surgical option for the treatment of large-volume benign prostatic hyperplasia.
