LONG-TERM OUTCOMES OF BILATERAL SAGITTAL SPLIT OSTEOTOMY FOR SKELETAL CLASS II MALOCCLUSION

Abstract

Objective: To evaluate the long-term stability of hard tissues, soft tissues, and patient satisfaction after BSSO mandibular advancement in the treatment of skeletal Class II malocclusion with mandibular retrusion.

Methods: This retrospective cohort study included 32 patients, comprising 18 females and 14 males, who underwent BSSO at Van Hanh General Hospital between 2018 and 2022. Long-term stability of hard tissues, soft tissues, and patient satisfaction after surgery were assessed.

Results: The mean mandibular advancement was 5.46 ± 1.81 mm. Immediately after surgery (T1–T2), SNB increased by 2.81°, ANB decreased by 2.96°, overjet decreased by 5.69 mm, and overbite decreased by 2.21 mm. Soft-tissue angles also showed significant changes, including N-A-Pog −4.77°, N’-Sn-Pog’ +6.01°, and N’-Pn-Pog’ +3.95°. During the T3–T4 period, no significant skeletal or dental relapse was observed. Soft-tissue changes in LL-E-line (−0.92 mm) and N’-Sn-Pog’ (+2.11°) were statistically significant. Early neurosensory disturbance occurred in 26/32 patients (81.25%); at T4, persistent paresthesia remained in 6/26 patients. The OQLQ score at T4 was 10.7 points.

Conclusion: BSSO mandibular advancement with rigid internal fixation demonstrated good long-term skeletal and dental stability, maintained improvement in lateral facial aesthetics, and was associated with favorable postoperative quality of life in patients with skeletal Class II malocclusion with mandibular retrusion.
