Objective: To evaluate the efficacy and safety of laparoscopic appendectomy in patients aged ≥80 years at Thong Nhat Hospital, and to identify factors associated with postoperative complications.
Methods: A cross-sectional descriptive study was conducted on 52 patients aged 80 years or older who were diagnosed with acute appendicitis and underwent laparoscopic appendectomy between January 2020 and October 2025. Data were collected from medical records and analyzed using SPSS version 26.0.
Results: The mean age was 84.1 ± 3.1 years, and 58% were male. The mean operative time was 79 ± 22 minutes, with a conversion to open surgery rate of 11.5%. The overall postoperative complication rate was 25%, with 8% classified as Clavien–Dindo grade III–IV. There were no intraoperative or postoperative deaths. Exploratory univariable analyses suggested that appendiceal perforation and conversion to open surgery were associated with a higher occurrence of postoperative complications.
Conclusion: Laparoscopic appendectomy in patients aged ≥80 years is safe and effective, with low morbidity and no mortality. Careful preoperative assessment and optimal management of risk factors are essential to improve surgical outcomes.
