A foreign body in the maxillary sinus for over 50 years is a rare nasal sinus foreign body that can cause chronic rhinosinusitis. A 74-year-old female patient presented to the ENT Department of Tra Vinh University Hospital with nasal congestion, bad breath, thick phlegm, and headache. Nasal endoscopy revealed a large, dark, hard, tender mass completely obstructing the right anterior nasal cavity. A CT scan showed a round, radiopaque mass measuring 2.47 x 3.19 x 2.54 cm in the right nasal sinus. The patient underwent endoscopic sinus surgery to remove the foreign body, believed to be a bomb fragment, from the maxillary and nasal sinuses. The patient recovered well. Endoscopic sinus surgery for removing maxillary sinus foreign bodies is an effective, gentle, and minimally invasive method.
