Abstract

Mục tiêu: Mô tả các biến cố tim mạch sau phẫu thuật ngoài tim ở người cao tuổi và mối liên quan đến các yếu tố tiền phẫu, trong đó có khả năng gắng sức của bệnh nhân.

Đối tượng và phương pháp: Nghiên cứu thuần tập tiến cứu 233 bệnh nhân trên 65 tuổi trải qua phẫu thuật ngoài tim tại Bệnh viên Bạch Mai từ tháng 1/2025 đến tháng 6/2025.

Kết quả: Tuổi trung bình của mẫu nghiên cứu là 75,45 ± 7,75. Trong 233 bệnh nhân, có 73,4% bị tăng huyết áp; 35,2% đái tháo đường (sử dụng Insulin); 22,7% có tiền sử suy tim sung huyết; 35,2% bị bệnh cơ tim thiếu máu cục bộ; 17,2% từng trải qua tai biến mạch não; 17,6% bị suy thận. Số bệnh nhân có METs < 4 chiếm, METs ≥ 4 chiếm 55,8%. Tầm soát sau phẫu thuật 30 ngày, có 14,6% bị tổn thương cơ tim chu phẫu; 7,7% bị nhồi máu cơ tim; 0,9% bị suy tim cấp; 0,4% bị loạn nhịp tim nặng; và 2,6% tử vong do tim. Với tổn thương cơ tim chu phẫu, METs có liên quan với OR là 6,2 (KTC 95%: 2,6-15,0; p < 0,001). Giá trị AUC để dự đoán nguy cơ bị tổn thương cơ tim chu phẫu khi bệnh nhân có METs < 4 đơn lẻ và khi kết hợp RCRI là AUC = 0,706; AUC = 0,804 (p < 0,05).

Kết luận: Tổn thương cơ tim chu phẫu là biến cố hay xảy ra sau phẫu thuật ngoài tim ở người cao tuổi, liên quan chặt chẽ với khả năng gắng sức trước phẫu thuật, nên cần kết hợp với chỉ số RCRI để tăng khả năng tiên lượng các biến cố tim mạch chu phẫu.
