Abstract

Mục tiêu: So sánh một số tác dụng không mong muốn khi phối hợp Levobupivacain 0,375% với Dexamethason 4 mg hoặc 8 mg trong gây tê cơ vuông thắt lưng dưới hướng dẫn siêu âm để giảm đau sau mổ lấy thai.

Đối tượng và phương pháp: Nghiên cứu can thiệp lâm sàng ngẫu nhiên, tiến hành trên 100 sản phụ được tê tủy sống để mổ lấy thai, giảm đau sau mổ bằng gây tê cơ vuông thắt lưng dưới hướng dẫn siêu âm với Levobupivacain 0,375% phối hợp với Dexamethason 4 mg (nhóm LD4, 50 sản phụ) hoặc Levobupivacain 0,375% với Dexamethason 8 mg (nhóm LD8, 50 sản phụ).

Kết quả: Tỷ lệ nôn, buồn nôn khác biệt giữa hai nhóm với p < 0,05: nhóm LD4 có 10%, nhóm LD8 không ghi nhận trường hợp nào. Tỷ lệ ngứa tương đương giữa hai nhóm LD4 và LD8: 26% mỗi nhóm. Không ghi nhận trường hợp nào xuất hiện các biến chứng như nhiễm trùng, bí tiểu, đau thượng vị…

Kết luận: Sử dụng phối hợp Dexamethason liều 4 mg hoặc 8 mg với Levobupivacain 0,375% trong gây tê cơ vuông thắt lưng dưới hướng dẫn siêu âm để giảm đau sau mổ lấy thai là phương pháp khá an toàn, tỷ lệ tác dụng không mong muốn thấp.
