13. RELATIONSHIP BETWEEN FRAILTY SYNDROME AND OSTEOPOROSIS IN ELDERLY PATIENTS AT THONG NHAT HOSPITAL

Abstract

Mục tiêu: Khảo sát tỷ lệ loãng xương và suy yếu trên bệnh nhân cao tuổi, và mối liên quan giữa suy yếu với loãng xương và thành phần cơ thể ở bệnh nhân cao tuổi.

Đối tượng: Bệnh nhân ≥ 60 tuổi điều trị tại khoa nội cơ xương khớp bệnh viện Thống Nhất. Phương pháp nghiên cứu: mô tả cắt ngang. Bệnh nhân được đo mật độ xương bằng DXA tại cổ xương đùi (CXĐ) và cột sống thắt lưng (CSTL), khối lượng cơ được đo bằng điện trở kháng sinh hoc (BIA). Bệnh nhân được đánh giá suy yếu theo thang điểm hội chứng suy yếu lâm sàng Canada (Clinical Frailty Scale- CFS).

Kết quả: Đối tượng nghiên cứu gồm có 200 bệnh nhân, tuổi trung bình 73,8 ± 8,9 tuổi, 72% nữ và 28% nam. Tỷ lệ loãng xương 55% tại CXĐ và 39,5% tại CSTL. Tỷ lệ suy yếu 27% nam (28,6%), nữ (26,45). Ở bệnh nhân suy yếu có tỷ lệ loãng xương tại cổ xương đùi cao gấp 2,58 lần so với bệnh nhân không bị suy yếu với OR=2,58, P<0,01. Tỷ lệ giảm khối lượng cơ, thiếu cơ và suy dinh dưỡng ở nhóm bệnh nhân suy yếu cao hơn nhóm không suy yếu và sự khác biệt có ý nghĩa thống kê với p<0,05. Khối lượng mỡ ở hai nhóm suy yếu và không suy yếu khác biệt không có ý nghĩa thống kê với p>0,05.

Kết luận: Tỷ lệ loãng xương và suy yếu cao ở bệnh nhân cao tuổi điều trị nội trú. Trên bệnh nhân suy yếu có tỷ lệ loãng xương tại cổ xương đùi, giảm khối lượng cơ, thiếu cơ và suy dinh dưỡng cao hơn nhóm không suy yếu.
