CAN THIỆP HỖ TRỢ ĐỐI VỚI SỰ CẢI THIỆN TÂM LÝ – XÃ HỘI LIÊN QUAN NGOẠI HÌNH SAU PHẪU THUẬT THẨM MỸ MÍ MẮT THEO THANG ĐIỂM DAS-24

Mục tiêu: Đánh giá sự thay đổi mức độ tự ti, lo âu và khó chịu tâm lý – xã hội liên quan ngoại hình sau phẫu thuật thẩm mỹ mí mắt theo thang điểm DAS-24 và xác định hiệu quả của quy trình can thiệp hỗ trợ đối với kết quả tâm lý – xã hội của người bệnh.

Phương pháp nghiên cứu: Nghiên cứu can thiệp lâm sàng có đối chứng được thực hiện trên 150 người bệnh có chỉ định phẫu thuật thẩm mỹ mí mắt tại Khoa Phẫu thuật Tạo hình Thẩm mỹ, Bệnh viện Đại học Y Hà Nội từ tháng 12/2023 đến tháng 2/2025. Nhóm can thiệp được áp dụng quy trình chăm sóc chuẩn hóa bao gồm tư vấn trước phẫu thuật, quản lý kỳ vọng. Kết quả được đánh giá bằng thang điểm Derriford Appearance Scale-24 (DAS-24) tại thời điểm trước điều trị, sau 1 tháng và sau 3 tháng. Hiệu quả can thiệp được phân tích theo phương pháp sai biệt kép.

Kết quả: Trước điều trị, điểm DAS-24 giữa nhóm chứng và nhóm can thiệp không có sự khác biệt có ý nghĩa thống kê (31,9±6,7 so với 32,5±7,8; p=0,534). Sau 1 tháng, điểm DAS-24 ở nhóm can thiệp thấp hơn nhóm chứng (26,0±6,0 so với 29,0±6,9; p=0,001). Sau 3 tháng, sự khác biệt tiếp tục duy trì với điểm DAS-24 ở nhóm can thiệp là 23,2±6,1 so với 26,2±6,6 ở nhóm chứng (p=0,0009). Hiệu quả can thiệp tuyệt đối theo phương pháp sai biệt kép là -3,69 điểm sau 1 tháng và -3,65 điểm sau 3 tháng. Hiệu quả can thiệp tương đối đạt 129,0% sau 1 tháng và 64,1% sau 3 tháng. Không ghi nhận mối liên quan có ý nghĩa thống kê giữa mức cải thiện DAS-24 với tuổi, giới, học vấn, tình trạng hôn nhân, nơi sống, điều kiện kinh tế, số lần khám và phương pháp tạo hình mí.

Kết luận: Phẫu thuật thẩm mỹ mí mắt giúp cải thiện rõ rệt mức độ tự ti, lo âu và khó chịu tâm lý – xã hội liên quan ngoại hình theo thang điểm DAS-24. Quy trình can thiệp hỗ trợ với tư vấn, chăm sóc và theo dõi hậu phẫu góp phần làm gia tăng hiệu quả cải thiện tâm lý sau điều trị.
