TỔNG QUAN VỀ KẾT QUẢ PHẪU THUẬT NỘI SOI MŨI XOANG ĐIỀU TRỊ VIÊM MŨI XOANG MẠN TÍNH Ở TRẺ EM

Mục tiêu: Tổng hợp và phân tích các bằng chứng hiện có về hiệu quả, tính an toàn và các yếu tố liên quan đến kết quả phẫu thuật nội soi mũi xoang điều trị viêm mũi xoang mạn tính ở trẻ em.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu tổng quan luận điểm (scoping review) được thực hiện theo định hướng PRISMA-ScR. Các nghiên cứu được tìm kiếm trên PubMed, Google Scholar, ScienceDirect, thư viện Trường Đại học Y Hà Nội và các nguồn tạp chí y học trong nước. Tiêu chuẩn lựa chọn gồm các nghiên cứu lâm sàng có toàn văn bằng tiếng Việt hoặc tiếng Anh, công bố từ 01/2010 đến 12/2025, có dữ liệu về kết quả phẫu thuật nội soi mũi xoang điều trị viêm mũi xoang mạn tính ở trẻ em.

Kết quả: Có 13 bài báo khoa học được đưa vào nghiên cứu với 1456 bệnh nhân. Tỷ lệ thành công chung của phẫu thuật dao động từ 62,5% đến trên 95%, trong đó nghẹt mũi là triệu chứng cải thiện rõ rệt nhất (65,3%–90,3%). Điểm Lund–Mackay giảm mạnh sau mổ và hiệu quả này được duy trì qua một số nghiên cứu theo dõi dài hạn, kể cả đến tuổi thành. Chất lượng cuộc sống được cải thiện rõ rệt thông qua các nghiên cứu sử dụng các thang điểm SN-5, GCBI và SNOT-22. Phẫu thuật có độ an toàn cao, biến chứng lớn chỉ ghi nhận trong 1/13 nghiên cứu (chiếm 0,3%), dính niêm mạc là biến chứng nhỏ thường gặp nhất (5,5% – 57,1%), đặc biệt cao ở trẻ ≤5 tuổi. Tỷ lệ phẫu thuật lại dao động từ 0% đến 15,5%, liên quan đến tuổi nhỏ, điểm Lund–Mackay trước mổ cao và tình trạng dị viêm mũi dị ứng.  

Kết luận: Phẫu thuật nội soi mũi xoang là phương pháp điều trị an toàn và hiệu quả cho trẻ em bị viêm mũi xoang mạn tính sau khi thất bại với điều trị nội khoa. Phẫu thuật cần được chỉ định đúng, tiếp cận toàn diện, ưu tiên kỹ thuật bảo tồn niêm mạc, theo dõi hậu phẫu chặt chẽ và kiểm soát các bệnh lý nền kèm theo để duy trì hiệu quả lâu dài.
