ĐÁNH GIÁ KẾT QUẢ ĐIỀU TRỊ VÀ KHẢ NĂNG TRỞ LẠI THỂ THAO SAU PHẪU THUẬT NỘI SOI TÁI TẠO DÂY CHẰNG CHÉO TRƯỚC KHỚP GỐI TẠI BỆNH VIỆN 199

Mục tiêu: Đánh giá kết quả lâm sàng và tỷ lệ trở lại thể thao (TLTT) sau phẫu thuật nội soi tái tạo dây chằng chéo trước (DCCT) bằng kỹ thuật “tất cả bên trong” (all‑inside).

Phương pháp: Nghiên cứu tiến cứu, mô tả loạt ca trên 92 bệnh nhân chấn thương DCCT tại Bệnh viện 199 (2022–2025). Đánh giá chức năng bằng thang điểm Lysholm, IKDC 2000, ACL‑RSI và ba bài kiểm tra chức năng (nhảy đơn một chân, nhảy 3 bước chéo, sức cơ tứ đầu) tại 12 và 24 tháng. Ngưỡng xác định trước: IKDC 2000 ≥85, ACL‑RSI ≥56, chỉ số đối xứng chi dưới (LSI) cho nhảy ≥90% và cho sức cơ ≥80%.

Kết quả: Tỷ lệ tốt và rất tốt theo Lysholm đạt 96,7% tại 24 tháng. Tỷ lệ TLTT thực tế là 45,7% (42/92) ở 12 tháng và 63,0% (58/92) ở 24 tháng. Chỉ 16,3% bệnh nhân đáp ứng đồng thời cả 5 ngưỡng. Trong hồi quy logistic, IKDC 2000 (OR=4,1; p=0,002) và ACL‑RSI (OR=4,8; p=0,001) là các yếu tố dự báo độc lập mạnh nhất.

Kết luận: Kỹ thuật all‑inside mang lại kết quả lâm sàng tốt. Tỷ lệ đáp ứng đầy đủ các tiêu chí TLTT tại 12 tháng còn thấp, đặc biệt về sức cơ tứ đầu và tâm lý, cần chiến lược phục hồi chức năng cá thể hóa kéo dài.
