Đặt vấn đề và mục tiêu: Bít tiểu nhĩ trái qua đường ống thông (LAAO) đang dần vượt ra khỏi vai trò ban đầu — một biện pháp dự phòng đột quỵ dành riêng cho bệnh nhân rung nhĩ không do bệnh van tim (NVAF) có chống chỉ định kháng đông — để trở thành lựa chọn cho một phổ bệnh nhân rộng hơn. Bài viết cập nhật đến tháng 7/2026 các tiến bộ về thiết bị, bằng chứng từ những thử nghiệm ngẫu nhiên có đối chứng (RCT) mới nhất, cùng những vấn đề còn bỏ ngỏ.

Phương pháp: Đây là một tổng quan tường thuật (narrative review), không phải tổng quan hệ thống. Chúng tôi tra cứu PubMed, Cochrane, Google Scholar và kỷ yếu các hội nghị (ACC, AHA, ESC, TCT) giai đoạn 2009–7/2026, ưu tiên RCT, phân tích gộp, registry lớn và các hướng dẫn; dữ liệu hội nghị đều được đối chiếu với bản công bố toàn văn.

Kết quả: Ba RCT đăng trên New England Journal of Medicine đóng vai trò trung tâm: OPTION (n≈1.600) cho thấy LAAO sau triệt đốt không kém hơn kháng đông và giảm chảy máu; CHAMPION-AF (n=3.000) chứng minh LAAO không kém hơn NOAC về hiệu quả (5,7% so với 4,8%) và vượt trội về chảy máu ngoài thủ thuật (10,9% so với 19,0%), dù khi gộp cả biến cố chu thủ thuật thì chảy máu nặng toàn bộ lại tương đương; còn CLOSURE-AF (n=912) không đạt tiêu chí không kém hơn so với điều trị nội khoa ở nhóm nguy cơ rất cao (P=0,44). Bên cạnh đó, ANDES (n=510) cho thấy kháng đông ngắn hạn an toàn hơn kháng kết tập tiểu cầu kép, và hướng dẫn SCAI/HRS 2025 đưa ra 8 khuyến cáo dựa trên phương pháp GRADE.

Kết luận: Vai trò của LAAO đang mở rộng trên nhóm bệnh nhân chọn lọc, nhưng lợi ích lệ thuộc rõ vào việc chọn đúng người bệnh và kinh nghiệm của trung tâm. Điều quan trọng là phải tách bạch nhóm chống chỉ định kháng đông với nhóm vẫn đủ điều kiện dùng DOAC, đồng thời từng bước xây dựng dữ liệu riêng cho Việt Nam.
