MỐI TƯƠNG QUAN GIỮA NỒNG ĐỘ FERRITIN HUYẾT TƯƠNG VỚI CÁC CHỈ SỐ HÓA SINH, HUYẾT HỌC TRÊN NGƯỜI BỆNH SỐT KÉO DÀI

Mục tiêu: Tìm hiểu mối tương quan giữa Ferritin huyết tương với các chỉ số hóa sinh, huyết học trên người bệnh sốt kéo dài điều trị nội trú tại Bệnh viện Bạch Mai. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 60 người bệnh sốt kéo dài điều trị nội trú tại Bệnh viện Bạch Mai từ tháng 06/2022 đến tháng 06/2023. Người bệnh được chia thành nhóm nhiễm trùng (n=36) và không do nhiễm trùng (n=24). Các chỉ số Ferritin, CRP, PCT, LDH, GOT, GPT, hồng cầu (RBC), tiểu cầu (PLT) được định lượng và phân tích mối tương quan Pearson bằng phần mềm SPSS 20.0. Kết quả: Ở nhóm nhiễm trùng, Ferritin tương quan thuận với PCT (r=0,51; p=0,001), GOT (r=0,58; p<0,01), GPT (r=0,38; p=0,021) và LDH (r=0,41; p=0,012). Ở nhóm không nhiễm trùng, Ferritin tương quan thuận với PCT (r=0,65, p = 0,001), GOT (r=0,61, p = 0,001), GPT (r=0,43, p = 0,034), LDH (r=0,53, p = 0,008) và tương quan nghịch với PLT (r=-0,59; p=0,002) và RBC (r=-0,42; p=0,04). Giá trị trung bình của LDH ở nhóm không nhiễm trùng là 460 U/L và ở nhóm nhiễm trùng là 291 U/L (p=0,023). Kết luận: Ferritin huyết tương có mối tương quan thuận với tổn thương tế bào gan (GOT, GPT), tổn thương mô (LDH) và tình trạng viêm (PCT) ở cả hai nhóm. Riêng ở nhóm không nhiễm trùng, Ferritin còn tương quan nghịch với hồng cầu và tiểu cầu, phản ánh tổn thương đa cơ quan.
