Objectives: Research characteristics of pathogenic bacteria, antibiotic resistant and using empirical-antibiotic therapy in the treatment of acute exacerbations of Chronic Obstructive Pulmonary Disease at the respiratory department of Trung Vuong hospital.
Methods: Adult inpatients diagnosed with acute exacerbations of COPD at the respiratory department of Trung Vuong hospital from 01/01/2024 to 31/12/2024.
Result: Elder patients group (≥60 years old) accounted for a large proportion (58.3%). The majority is severe COPD (66.7%). Most of patients in our research are male (72.2% vs 27.8%). P.aeruginosa (33.3%), A.baumannii (22.2%) and K.pneumoniae (13.9%) are the main causes of acute exacerbations of COPD. The antibiotic resistance rate of each bacteria was high, such as P.aeruginosa is resistant to many antibiotics from 40% to 60% include Ciprofloxacin, Amikacin, Cephalosporin and Carbapenem. A.baumannii has the highest rate of antibiotic resistant (almost >60%). K.pneumoniae is also strongly resistance to lots of antibiotic (40-90%). Antibiotics commonly used in the treatment of acute exacerbations of COPD are levofloxacin (47.2%), Cefoperazone (19.4%) and Meropenem (16.7%).
Conclusion: Leading causes of acute exacerbations of COPD at the respiratory department of Trung Vuong hospital are P.aeruginosa, A.baumannii, and K.pneumoniae. Rate of antibiotic resistant is high level. Empirical-antibiotic therapy is used appropriately in accordance with the treatment protocol for infectious exacerbations of COPD.
