MICROBIOME TRONG SINH SẢN VÀ CHỨC NĂNG TÌNH DỤC NAM: TỪ TRỤC RUỘT-TINH HOÀN ĐẾN ỨNG DỤNG LÂM SÀNG

Microbiome ngày càng được xem là một thành tố quan trọng trong điều hòa chức năng sinh sản và sinh lý tình dục nam giới thông qua các cơ chế miễn dịch, chuyển hóa và nội tiết. Bài tổng quan tường thuật này nhằm cập nhật vai trò của hệ vi sinh đường ruột và hệ vi sinh đường sinh dục nam trong sinh tinh, chất lượng tinh trùng, testosterone và rối loạn chức năng tình dục nam, đặc biệt là rối loạn cương dương.

Các tài liệu được tổng hợp từ các bài tổng quan, nghiên cứu quan sát, thử nghiệm ngẫu nhiên có đối chứng, thử nghiệm ngẫu nhiên tự nhiên bằng di truyền học và guideline hiện hành, chủ yếu công bố trong giai đoạn 2022–2026. Các bằng chứng hiện nay cho thấy mất cân bằng hệ vi sinh có liên quan đến giảm sinh tinh, bất thường tinh dịch đồ, tăng tổn thương DNA tinh trùng, viêm mạn tính mức độ thấp và rối loạn chức năng nội mô liên quan đến rối loạn cương. Một số nghiên cứu di truyền học gần đây cũng gợi ý rằng một số nhóm vi khuẩn đường ruột có thể liên quan trực tiếp đến nguy cơ rối loạn cương dương. Bên cạnh đó, probiotic và synbiotic bước đầu cho thấy khả năng cải thiện một số chỉ số tinh dịch và giảm stress oxy hóa ở một số nhóm bệnh nhân chọn lọc.

Tuy nhiên, phần lớn dữ liệu hiện nay vẫn còn không đồng nhất và chủ yếu dựa trên nghiên cứu quan sát hoặc mô hình tiền lâm sàng. Ở thời điểm hiện tại, microbiome nên được xem là một lớp thông tin sinh học bổ sung trong nam khoa hơn là một đích điều trị chuẩn hóa. Tối ưu hóa lối sống và chuyển hóa vẫn là chiến lược có cơ sở bằng chứng mạnh nhất nhằm hỗ trợ nền vi sinh khỏe mạnh ở nam giới.
