Đặt vấn đề: Dị vật sắc nhọn đường tiêu hoá có thể gây thủng ruột và viêm phúc mạc; thủng ruột do nuốt nhiều dị vật sắc ở thiếu niên hiếm gặp.

Ca bệnh: Bé gái 13 tuổi vào viện vì đau bụng, nôn; khám ghi nhận viêm phúc mạc khu trú. CT-scan phát hiện nhiều dị vật dạng thanh trong ruột non. Bệnh nhân có tiền căn thủng hồi tràng do tăm tre và ăn nhanh, mất tập trung khi ăn. Phẫu thuật thám sát ghi nhận thủng hỗng tràng cách góc tá–hỗng tràng 20 cm do mảnh xương gà xuyên qua; lấy tổng cộng 7 mảnh xương và khâu phục hồi lỗ thủng. Hậu phẫu thuận lợi; ăn lại ngày 3, xuất viện ngày 6; tái khám 1 tháng không biến chứng; được tư vấn thay đổi hành vi ăn uống.

Kết luận: Thủng ruột do dị vật sắc ở trẻ lớn hiếm nhưng nguy hiểm. Chẩn đoán sớm bằng CT-scan, phẫu thuật kịp thời với thám sát toàn bộ ruột và tư vấn hành vi giúp hạn chế bỏ sót dị vật và phòng ngừa tái diễn.
