12. BÁO CÁO CA LÂM SÀNG GÂY MÊ TRONG PHẪU THUẬT NÃO TỈNH

Phẫu thuật não tỉnh kết hợp theo dõi điện sinh lý trong mổ giúp lập bản đồ vỏ não, loại bỏ tối đa vùng não tổn thương mà vẫn bảo tồn được chức năng thần kinh. Để phẫu thuật não tỉnh thành công, cần có sự hợp tác của người bệnh, sự giao tiếp trước và trong mổ, sự đồng thuận giữa các nhà phẫu thuật, gây mê và đội ngũ nhân viên phòng mổ. Chưa có sự đồng thuận về phương pháp vô cảm tối ưu trong phẫu thuật não tỉnh. Dựa trên đặc điểm của từng bệnh nhân và phẫu thuật, chúng tôi đã chọn phương pháp mê-tỉnh-mê (Asleep-Awake-Asleep) cho ca phẫu thuật cắt bỏ tổn thương não gây động kinh, và phương pháp Awake-Asleep trong ca đặt điện cực não sâu. Phong bế thần kinh da đầu đóng vai trò quan trọng trong quản lý đau phẫu thuật não tỉnh.
