16. ĐÁNH GIÁ KẾT QUẢ PHẪU THUẬT THAY KHỚP HÁNG TOÀN PHẦN VỚI ĐƯỜNG MỔ ABMS

Đặt vấn đề: Phẫu thuật thay khớp háng đường ABMS (anterior-based muscle-sparing) cho thấy phẫu thuật ít xâm lấn, hiệu quả giúp bệnh nhân phục hồi nhanh, an toàn, giảm tỉ lệ trật khớp sau mổ.

Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu theo dõi 44 ca thay toàn bộ khớp háng đường mổ ABMS được thực hiện tại bệnh viện đa khoa Tâm Anh TPHCM từ 5/2023 đến 02/2024 với thời gian theo dõi ít nhất 6 tháng.

Kết quả: Thời gian mổ trung bình 77.12 ± 17.82 phút, chiều dài vết mổ 8.65 ± 1.87 cm, lượng máu mất trong mổ 269.51 ± 85.78 ml, ∆Hgb sụt giảm sau mổ 1.86 ± 0.89 g/dL, 01 trường hợp truyền máu, thời gian nằm viện 3.4 ± 0.9 ngày. Thang điểm Harris trước mổ, sau mổ 3 tháng và 6 tháng lần lượt là 43.5 ± 6.8, 83.5 ± 2.8 và 91.1 ± 3.8, 01 trường hợp trật khớp háng (2.27%), 2 trường hợp gãy mấu chuyển trong mổ (4.54%)

Kết luận: Phẫu thuật thay toàn bộ khớp háng với đường mổ ABMS bước đầu cho thấy kết quả phục hồi sau mổ tốt, ít mất máu, thời gian nằm viện ngắn và tỉ lệ biến chứng, trật khớp sau mổ thấp, phương pháp dễ thực hiện với bàn mổ thường và sử dụng được nhiều loại thiết kế khớp nhân tạo không cần trợ cụ chuyên biệt.
